You do not need to be a Costco member in order to book an eye exam with us! If though we are located within Costco, we are an independent practice with our own team of highly trained technicians and board certified doctors.

Please bring any previous glasses and/or contact lens prescription and medical records/eye diagnoses (if any). Please note: If you are an EXISTING contact lens wearer, you need to bring your previous contact lens prescription in order to be considered an existing contact lens wearer otherwise we will fit you as a NEW contact lens patient.
We do not perform dilation during our telemedicine exams, as we use the advanced Optomap wide-field retinal imaging to assess the retina and periphery in superior detail. However, we do recognize the importance of dilation and recommend that you undergo dilation within the year.
Simply join and sign in to your patient portal - all your eye exam records, prescriptions and invoices will be in your portal for you to access at any time immediately after your eye examination.
The username/password to your portal will be given to you at the time of your eye exam.
Of course! Simply have a conversation with your eye doctor and we will choose a contact lens brand that best fits your needs.
Unfortunately we are unable to see NEW Contact lens wearers at this time.
Please Note: If you have never worn contacts before OR your contact lens prescription is more than 2 years old, you are considered a NEW Contact Lens wearer.
Please bring your previous contact lens prescription with you to your eye exam.
What happens if the doctor diagnoses a medical issue during my telemedicine eye exam?
If there is an active medical issue that prevents the accurate determination of a glasses/contact lens prescription, the exam will be stopped and the patient will be referred to a local ophthalmologist to resolve the medical issue first. If this occurs, there will be no charge to you for the eye exam.
Of course! As part of Connecticut’s DMV requirements, a visual field test will be performed (at a small additional fee of ) during your eye examination, which will allow the remote doctor to accurately complete the DMV form provided.
