FREQUENTLY ASKED QUESTIONS
  • Do I need a Costco Membership to book an eye appointment? (Quick Answer - No)
    You do not need to be a Costco member in order to book an eye exam with us! If though we are located within Costco, we are an independent practice with our own team of highly trained technicians and board certified doctors. We welcome all patients - with or without a Costco membership!
  • Do I need an appointment or can I walk-in?
    We accept both! An appointment may lessen your wait time but we would love to service all your eye care needs whether you book an appointment or walk in!
  • Do you accept same-day appointments/walk-in?
    Yes we do! Come on in - we would be honored to take care of all your eye care needs.
  • Do you accept vision insurance?
    We accept Eyemed insurance for Costco Employees only at this time. Payment forms accepted include cash, check and credit card payments.
  • What age group(s) do you accept?
    We accept all patients ages 10 and up!
  • What should I bring to my eye exam?
    Please bring any previous glasses and/or contact lens prescription and medical records/eye diagnoses (if any). Please note: If you are an EXISTING contact lens wearer, you need to bring your previous contact lens prescription in order to be considered an existing contact lens wearer otherwise we will fit you as a NEW contact lens patient.
  • Will I be dilated during my telemedicine eye exam?
    We do not perform dilation during our telemedicine exams, as we use the advanced Optomap wide-field retinal imaging to assess the retina and periphery in superior detail. However, we do recognize the importance of dilation and recommend that you undergo dilation within the year.
  • How can I access my glasses/contacts prescription and medical records?
    Simply join and sign in to your patient portal - all your eye exam records, prescriptions and invoices will be in your portal for you to access at any time immediately after your eye examination. The username/password to your portal will be given to you at the time of your eye exam.
  • During a contact lens exam, can I change my brand of contact lenses?
    Of course! Simply have a conversation with your eye doctor and we will choose a contact lens brand that best fits your needs.
  • I have never worn contact lenses before, can I be seen for a NEW Contact lens fitting?
    Unfortunately we are unable to see NEW Contact lens wearers at this time. Please Note: If you have never worn contacts before OR your contact lens prescription is more than 2 years old, you are considered a NEW Contact Lens wearer. Please bring your previous contact lens prescription with you to your eye exam.
  • What happens if the doctor diagnoses a medical issue during my telemedicine eye exam?
    At Jaya Vision PC, we conduct routine comprehensive eye exams/contact lens exams. If a medical eye-related condition is diagnosed, it may require further specialized testing and follow-up care. In such an event, the remote doctor will thoroughly explain the eye exam findings and refer you to local Ophthalmologists for further follow-up care.
  • What happens if the doctor does not finalize a glasses prescription due to a medical issue?
    If there is an active medical issue that prevents the accurate determination of a glasses/contact lens prescription, the exam will be stopped and the patient will be referred to a local ophthalmologist to resolve the medical issue first. If this occurs, there will be no charge to you for the eye exam.
  • Can I have my DMV form filled out at the end of the exam?
    Of course! As part of Connecticut’s DMV requirements, a visual field test will be performed (at a small additional fee of $20) during your eye examination, which will allow the remote doctor to accurately complete the DMV form provided.
  • I am an employee of Costco. What are the prices for the glasses and/or contact lens exam?
    We do our absolute best to accommodate all Costco employees! Please call our office at 203-775-4337 or email us at infojayavision@gmail.com to learn of our exam prices for Costco Employees (all employees must present to the eye exam with an employee number/ID in order to qualify).
